If requested, I will be given a repair estimate range based upon the information I have provided on this form prior to a pickup or service call.

I will be contacted prior to work completion if repairs are to exceed this range estimate.

If I require a detailed estimate, equipment must be physically inspected prior to any work performed.

PAYMENT IS DUE AT THE TIME SERVICES ARE RENDERED.

IF NO ONE WILL BE PRESENT FOR THE SERVICE CALL, A DEPOSIT WILL BE REQUIRED PRIOR TO SERVICE CALL.

There is a $50.00 minimum charge that applies to all Pickup / Delivery and On Site Service Calls

$25.00 of this charge will be applied to any repair charges and $25.00 is a trip charge.

If I choose not to have any work completed upon a pickup or an onsite visit, I will still OWE the minimum charges regardless of any or no work performed

If any work required to properly evaluate and provide a repair estimate should exceed the minimum charge, then that amount will be due and payable in lieu of the minimum charge
